2. Insert the authorization form and your check or money order for $100 payable to Funeral At Sea in the outer cremated remains shipping container.  Mail via U.S.P.S. Registered Mail to:

Your funeral at sea will be performed within 30 days and certificate documenting the: day, time and location will be sent to you.
